Lifeplan provides mental health and disability services throughout Perth, to people of all ages. We are a WA NDIS service provider and pride ourselves on working collaboratively with local co-ordinators, planners, guardians and accommodation providers. We have been a leading not-for-profit disability service organisation in the Perth community for the past 18 years, providing positive disability support and mental health services.
