Following a stay in hospital, people sometimes need extra support to recover and make decisions about the best place to live long term. Brightwater's transition care gives older people the time and support to make these important decisions with a focus on helping them to improve their confidence and functional ability.

Brightwater provides both in home and residential transition care for older people following a stay in hospital so they can return home, or await suitable permanent high or low care accommodation.

At Brightwater, care is at our core. As a not-for-profit company, we support West Australians from all walks of life, particularly seniors and younger people with acquired disabilities. We have 11 residential aged care homes across Perth, and also support seniors in their own homes and in our retirement village. We also provide disability and NDIS services to young people with complex disabilities.
